Independent quality management from first mockup to final punch, run by people who do not also carry the schedule — because the person under pressure to finish should not be the person deciding it is finished.
The structural conflict of interest in construction quality is that the person judging the work usually also owns the date. We separate the two: quality reports independently of the project team.
Most rework traces back to first-work-in-place that nobody looked at hard enough. Getting the first bay, the first wall section, the first pour right — and reviewing it before the trade repeats it forty times — prevents more rework than any amount of inspection afterwards.
